Job Description:
Diabetes Care & Education SpecialistJOB SUMMARY
The Certified Diabetes Care and Education Specialist (CDCES) plans,
directs, and provides nutritional diabetes care to patients in the
outpatient setting. Provides medical nutrition therapy and diabetic
education to patients and families or significant others to improve
nutrition status and diabetes management. Communicates care plan
with interdisciplinary team.
RESPONSIBILITIES
Essential Functions
Demonstrates a service orientation and commitment to the mission of
Swedish Hospital, Part of NorthShore, and adheres to all
responsibilities and standards of the organization.
1. Plans, directs, and provides nutritional diabetes care to
patients.
2. Provides initial and follow-up diabetes education sessions.
3. Conducts diabetes and/or nutrition education classes when
needed.
4. Electronically documents all aspects of diabetes care.
5. Participates in conferences and meetings as appropriate;
represents the work unit on organizational initiatives.
6. Attends special meetings and rounds as assigned.
7. Assists with quality improvement audits and projects as
assigned.
PATIENT CARE/AGE SPECIFIC RESPONSIBILITIES AND QUALIFICATIONS
Independently anticipates and always responds appropriately to the
age related patients' psychosocial and physical needs.
Independently obtains and integrates clinical information in terms
of age and developmental needs of patients and responds
appropriately.

* Computer system access is assigned based on director's access
level request and on the minimum necessary as described in this job

QUALIFICATIONS / JOB REQUIREMENTS
* Bachelor of Science degree
* Ability to communicate effectively with a diverse patient
population, interdisciplinary medical team
* Working knowledge of MS Office applications (Word, Excel),
intranet/internet, and web-based portal functionality
* Ability to chart and explain patient needs through assessment
* Ability to interact positively and effectively with multiple
departments
* Ability to use good judgement and utilize independent
decision-making skills to effectively problem solve and resolve
issues
* EPIC experience preferred
LICENSE/CERTIFICATION
* Current Illinois state license as a Licensed Dietitian
Nutritionist or Registered Nurse
* If Registered Dietitian: Current registration as a Registered
Dietitian by the Commission on Dietetic Registration and current
certification by the Certification Board for Diabetes Care and
Education as a Certified Diabetes Care and Education Specialist
(CDCES).
* If Registered Nurse: Current Certification by the Certification
Board for Diabetes Care and Education as a Certified Diabetes Care
and Education Specialist (CDCES).

ENVIRONMENT
* Sits at desk 50-75% of day; On feet and walking 25-50%
* Exposed to patients on respiratory isolation
* Potential for exposure to chemicals, airborne diseases, blood and
bodily fluids.
