Job Description:

The Specialized Resource Care Manager will work collaboratively with the Robert R. McCormick Foundation Center for Hope and Equity manager and the I.D.E.A. Council, whose acronym stands for Inclusion, Diversity, Equity and Action, to identify and reduce health care disparities affecting patients.
The Robert R. McCormick Foundation Center for Hope and Equity is a newly created initiative that brings together various components of Shirley Ryan AbilityLab's equity initiatives, including care management, vocational rehabilitation, staff training, staff hiring and assessment of patients' health-related social needs.
The Specialized Resource Care Manager will partner with the Inpatient Care Manager to provide additional support to underserved patients with complex discharges. The Specialized Resource Care Manager will communicate with internal clinical teams to assess, develop, coordinate, advocate, and monitor a care plan that utilizes the patient's available resources in the most appropriate and cost-effective manner. Responsible for the overall knowledge of the patient's plan of care, goals, and objectives to help connect the patient to resources to best support health equity and inclusion.

Council priorities are centered around health equity, staff recruitment, education and career development.


Principal Responsibilities
The Specialized Resource Care Manager:

  • Coordinates necessary resources for patients from underserved communities who have difficulty accessing medical care, maximizing insurance covered benefits, and connection to community service support.
  • Utilizes Social Determinants of Health screening tools to determine which patient populations are most vulnerable and work to provide curated resources to help alleviate health disparities.
  • Performs psychosocial assessments and provides ongoing support to patients, families, and clinical team throughout stay.
  • Monitors patient and family goal attainment relative to the interdisciplinary treatment goals during the clinical team conferences.
  • Maintains communication with external case managers, external providers (including NMH, CMH, UCMC and private physician groups) and community partners to ensure smooth transition at time of discharge.
  • Negotiates any conflict arising with a third party payor representative on behalf of the patient in relation to connection to community resources.
  • Continues to work with vulnerable populations post-discharge to ensure the patient is able to access follow up medical care, post-acute services, and community resources.
  • Participates in planning, development, and execution of quality improvement initiatives that positively impact individual staff, team, and hospital wide programming to foster equity and inclusion for all.
  • Performs all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLab.



    Reporting Relationships

  • Reports directly to the Director, IP Care Coordination and Resource Utilization.
