Job Summary:
WELLBE INTRODUCTIONThe WellBe care model is a Physician Led Advanced Practice -clinician -driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients. - This population is typically underserved and very challenged with access to care. - - To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care - from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care. - WellBe's physician/advanced practicing clinician led geriatric care teams' partner with the patient's primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum. -GENERAL SUMMARYThe Clinical Project Manager participates in cross functional internal and external teams to define, create, pilot, and launch various projects, and programs that benefit the WellBe Senior Medical model. As the quality program manager, you will function as a "SME" leading initiative in a fast-paced environment that leads to progressive decision making based on project/program risk, timelines, budget, and on time deliverables. A major objective for this position is to lead an effort in ensuring the scale and replication of the WellBe Model in delivering high quality care. -This will require time management, through communication, agility, organizational excellence, reporting, high degree of healthcare knowledge, and finally project/program effectiveness.

Job Description:
SKILLS & COMPETENCIES

Independently leads and motivates teams on internal and external projects in a fast-paced start-up environmentFunction as a subject matter expert (SME) leading effective coordination between organization/department process flows, stand-ups, retrospectives, sprint reviews, design, and planning.Define project scope, draft statements of work (SOWs), estimate budgets, and produce timelines that align to client and business needsLeads on-time, on-budget delivery of projects consistent with "On Time/On Budget" program management.Ability to anticipate issues for clinical teams and work toward a solution by building consensus within the project team.Participates in projects across the organization and be ultimately responsible for the delivery of high-quality deliverables that power client satisfaction, meet business requirements, and balance client economicsInterface heavily with marketing, technical, and creative senior leads to assure that the solution is understood and addresses the customer's business requirementsManage the execution of work, which includes tracking team progress, routing deliverables for approval, monitoring scope, and analyzing performanceActively monitor project risks/issues, escalate in a timely manner, and drive teams to effective solutionsIn collaboration with the department heads and team members develop, implement, and monitor an integrated operational plan that supports the successful execution of project/program strategy to meet goals for assigned project(s) in accordance with the project goalsDevelop and manage a detailed project plan(s) that integrates key activities, timelines, resources, budget, and track the critical path for assigned project(s).Ensure uniform processes/documentation between projects including project meeting organization, issuing agendas, documenting project team meeting minutes, tracking, reporting progress on projects and proactively managing follow up and resolution on action items.Promotes quality and performance standards for care deliveryMonitors and ensures clinical quality and clinical performance goals are maintained across the organizationHelp develop and implement clinical initiatives to ensure standardization goals are metFacilitate coordination of the project activities between functional areas and work with senior management to define roles and responsibilities of team members.Collaborate with team members to collect and maintain project-related information and prepare for presentations, proposals, plans, updates, issues, recommendations to senior management, and collaborative partners as needed.Facilitate decision-making process within the team.Foster open dialogue within the team and build positive team spirit to overcome obstacles.Facilitate resolution of conflicts between functional groups and external collaborators while maintaining objectivity.Support contractual and intellectual property requirementsOther tasks needed to accomplish team's objectives/goals

Job Requirements:
Educational Requirements:
BS/BA in relevant program, or equivalent work experience5+ years of project management experience 5+ years' experience in healthcare 5-7+ years applicable experience as a Project Manager Solid understanding of project management methodology, including the ability to identify and resolve issues, manage risk, and develop detailed work plans and specifications -Required Skills and Abilities:
Experience working as a project manager on external client facing projectsHealthcare experience is requiredExperience in start-up or fast paced dynamic environmentAbility to multi-task and manage competing prioritiesExperience interfacing with clients and partner vendors on various project deliverablesDemonstrated success managing complex projects or multiple smaller projects encompassing Quality, Clinical Operations, Clinical Development, procurement, and other product launchesMust be able to work in a matrix environment with experience managing external collaborations and multiple partnerships in academic, small and large healthcare insurance companies including healthcare providers.Ability to build strong multidisciplinary cross-functional teams and influence othersMust have the ability to motivate others, prioritize multiple tasks, and maintain positive interpersonal working relationshipsExcellent written and verbal communication skills are required. Must be able to communicate effectively with all levels of the organizationExcellent understanding and proficiency with Microsoft Office including strong MS Project skills and other project management software and standard Project Management tools and methodologiesPMP certification desirableSupervisory Responsibility: No supervisory responsibilities. -Travel requirements: Travel may be required up to 25% locally or nationally -Work Conditions: Ability to lift to 20lbs. - Moving lifting or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs. -
