This role provides patient care through a diverse role as a
clinician, educator, care coordinator and advocate, and for
providing education to various healthcare professionals. The role
will work to assess and treat patients and their family members in
a multidisciplinary outpatient practice setting. The Social Worker
will provide the necessary counseling and support to help stabilize
the family and their patient. Insight and direction for patients
will be offered to ensure patients are connected to other services
and support in the community. The Social Worker works in
collaboration with psychiatrists, psychologists, counselors and
other members of a multidisciplinary team. Care will be focused on
specific assigned patient populations. Exemplifies the Rush

policies and procedures, including complying with all Rush

Collaborates with team to identify patients in need of social work
and behavioral health services.
Reviews comprehensive risk assessments to develop plan of care and
address barriers with the patient, patient's family, and care team
Provides psychosocial education, supportive counseling,
anticipatory guidance, and linkage to community resources during
both individual and group sessions for patients and families as
indicated by comprehensive assessment.
Advocates within complex medical and community-based service
systems to improve patient access to needed services.
Leads collaboration with school behavioral health and
social-emotional learning teams.
Facilitates communication between patients, care team and
community-based providers to ensure continuity of care.
Contributes to development and documentation of individualized care
for identified patients, focusing on specific psychosocial
Provides educational materials and resources to patient/family that
are in a format which is specific to the patient/family learning
abilities and language preference.
Effectively collaborates with inter-professional healthcare teams
within and outside of the Rush system.
Works to improve identified CBP outcomes.
Participates in quality improvement initiatives. Activities may
include data collection, chart review, inter-professional
collaboration, analysis of patient data, and care team meetings
Attends individual and group supervision meetings for outpatient
social workers as well as relevant social work trainings.
Interviews, assesses and evaluates clients and families who present
themselves for care; provides professional outpatient counseling
and treatment with the goal of maximizing personal/family
Prepares intake summaries, treatment plans and case summaries while
maintaining on-going confidential records.
Charts member's treatment and progress in accordance with state and
other regulations and in keeping with accepted standards.
Develops, implements, and evaluates treatment approaches for
clients and affected family members, including individual, family,
group or intensive therapeutic modalities.
Coordinates internal and external educational activities and
training programs to support recruitment initiatives and goals of
the education committee.
Performs related duties as required.
Required Job Qualifications:
Master's degree in Social Work.
Current LSW licensure in the State of Illinois.
Two years of experience providing social services in healthcare or
