Position#Purpose:# The Intake Nurse is responsible for serving as a
member of the team that manages front-door operations for the
Psychiatry Department.# Referrals are made to the department from
the hospital#s emergency department, other hospital emergency
departments, the outreach team, community agencies and direct
referrals from staff physicians.# Screening is conducted to ensure
that an individual is appropriate for the services offered.# All
individuals presenting to the hospital#s ED and those referred from
other hospitals or facilities, are evaluated to determine the
intensity and severity of the presenting symptoms so that
appropriate safety measures are recommended.# Intake staff include
registered nurses, licensed counselors, and other staff deemed
appropriate to evaluate patients with serious and persistent mental
illness. The specific responsibilities of the Intake Nurse include
review of the incoming medical and legal paperwork, assessment of
psychiatric and medical issues, coordination with the Intake
Counselor to discuss the extent to which the individual meets
admission criteria, contact with the on-call psychiatrist or APRN
to discuss the case and facilitation of the nurse-to-nurse report.
# Essential Duties: Evaluates the appropriateness of the
prospective admission based upon the admission guidelines in
coordination with the Intake Counselor. Reviews the incoming
paperwork with reference to the physical and behavioral symptoms as
outline by the emergency department or other referral sources.
Highlights any issues that would serve as exclusionary criteria.
Discusses the case with the floor nurse who will be receiving the
admission with reference#to bed availability and unit acuity.
Assuming that the referral meets admission criteria, the Intake
Nurse will contact the psychiatrist on call to discuss the case,
receive an admission order and medication orders#as appropriate.
The Intake Nurse will contact the referring facility, conduct a
nurse-to-nurse interview, and#coordinate with the unit regarding
when the patient can be sent to the floor or the facility. #
Marginal Duties: Collaborates with team members by sharing relevant
information, demonstrating initiative, and supporting team
decisions as well as the work life of the department. Performs both
essential and marginal job functions in a safe manner as identified
by Saint Anthony Hospital and the respective department while
adhering to the Hospital#s confidentiality norms and standards.
Participates in performance activities including problem
identification, data collection, solution selection, implementation
and evaluation.# Maintains professional knowledge base through
literature review, seminars, workshops and meetings throughout the
calendar year.# Participates in professional association activities
to assist in meeting CE requirements. # General Job Requirements:
Bachelor#s degree in nursing (specialization in psychiatric nursing
preferred). A minimum of 2 years serving as a RN#on an inpatient
adult psychiatric unit.# Experience with the intake process
strongly preferred.# Bilingual Spanish a plus, but not required
