Transitions Care is seeking an Advance Practice Nurse (APN) or Physician Assistant (PA) to join our rapidly growing, innovating company that provides high quality health and wellness care to homebound patients in the community setting. This position is flexible and has the option for 1099 contract or W2 FT status with benefits. Transitions Care NPs/PAs are an integral part of our team. NPs/PAs evaluate and monitor patients independently with oversight of a collaborating physician. We partner with a network of specialists and hospitals for specialty and acute care. Transition's goal is to empower our patients and families by providing the tools and support necessary to embrace life.


  • The Advanced Practice Nurse/Physician Assistant reflects the mission and values of Transitions
  • Assure compliance with local, state, and federal laws, Medicare regulations, and HIPAA.
  • Accountable for direction and oversight of the care and treatment of palliative and hospice patients
  • Will implement the plan of care to meet the patient's medical needs, assess and reevaluate the patient's medical needs on a regular basis, and manage related expenditures in a fiscally responsible manner.
  • Local travel is necessary into patients' homes and facilities
  • Assumes responsibility for meeting regulatory and other mandatory requirements
  • Promote exemplary customer service to providers, facility personnel, patient, and patient family at all times
  • Reviews and interprets diagnostic results in a timely manner
  • Prepares and maintains clinical documentation utilizing company HER
  • Routine visits focusing on primary care and prevention
  • Care coordination with home health, specialists, facilities, and clinical coordinators
  • Emergency responder to all on-call visits as needed for case managers and clinical coordinators
  • Routine visits focusing on symptom management and quality of life
  • Valid driver's license and reliable transportation that is insured in accordance with state and/or Transitions requirements
  • Ability to utilize the necessary equipment such as BP monitor, pulse oximeter, otoscope, etc.
  • For 1099 PRN/part time- 4 mandatory scheduled shifts per month


  • A Master's Degree and Certification
  • CHPN certification preferred
  • An Active DEA license with no restrictions
  • A current license to practice medicine in the state and Controlled Substance
  • Valid driver's license and reliable transportation that is insuring in accordance with state and/or Transitions requirements
  • Demonstrate excellent observation, assessment, nursing judgement and communication skills
  • Self-directed with the ability to work with minimal supervision
  • Ability to work 8-10 hours shift(s)
  • Years of Experience: 1 year of home care or hospice experience (preferred)
