Duties
The VISN 12 Social Worker (Whole Health) Supervisor is a licensed
independent practitioner and holds board certification as a Health
and Wellness Coach or will attain within 180 days of hire. The
Social Worker (Whole Health) Supervisor's practice includes the
identified dynamics of Whole Health per Office of Patient Centered
Care and Cultural Transformation (OPCC &CT), Motivational
Interviewing techniques, as well as evidenced-based behavioral
change theories and conceptual frameworks to support optimization
of Veterans' health through a combination of behavior change and
personal reflection. This is done either through virtual individual
or group interactions. Social Worker (Whole Health) Supervisor
adheres to professional and ethical standards as defined by their
licensed practice and the tenets of Whole Health Coaching. Social
Worker (Whole Health) Supervisor will work closely with Clinical
Resource Hub leadership and staff, VISN 12 Whole Health Program
Manager and Office of Patient Centered Care Field Implementation
Team Consultants.

Major Duties
- Provides Whole Health program specific administrative and
clinical oversight of Tele-Whole Health Hub that provides virtual
Whole Health, Complementary and Integrative Health, and health
coaching to Veterans across VISN 12. This may include, but is not
limited to, developing and monitoring WH Hub staff compliance with
practice, developing standards and guidelines for documentation,
running reports and analytics of Tele-WH utilization and patient
outcomes.
- Exhibits the ability to articulate the philosophy and goals of
Whole Health in clinical and non-clinical settings.
- Ability to provide training, orientation, and guidance related
to Whole Health clinical practice to multidisciplinary
staff
- Provides specialized consultation to colleagues, clinical and
non-clinical staff related to the psycho-social assessment and
treatment utilizing a Whole Health and patient centric approach to
care
- Provide guidance and supervision to Tele-WH Hub health coaches
to ensure interventions and documentation are appropriate and
within scope of practice.
- Provide direct coaching support to Veterans with complex
circumstances who are actively engaged in PACT and/or Mental Health
under the Clinical Resource Hub. May be sole practitioner in Whole
Health clinical coaching.
- Partner with Veterans to establish a therapeutic relationship
where readiness for change will be assessed and documented. The
Veterans' Mission, Aspiration and Purpose (MAP) will be identified
along with addressing the components of health and
well-being.
- Partner with Veterans with complex needs as they continue to
utilize self-discovery/exploration and action planning as they
craft SMART (Specific, Measurable, Action oriented and Timed) Goals
and/or shared medical goals and measure against self-expressed
desired outcomes. Experience in assessing suicide risk and other
risk and completing risk assessment and safety plans when
indicated.
- Refer Veterans to other clinical team members, as appropriate.
Ensure other treating providers are aware of Veterans' MAP and
Personal Health Plan goals
- Demonstrate leadership skills by utilizing problem solving
skills, to resolve system and/or process problems that impact
Veteran care, staff performance and system improvements. Serve as a
mentor/trainer to assist employees in developing their skills
related to WH Coaching and WH principles
- Develops curriculums for training of clinical and non-clinical
staff. This includes trainings related to Whole Health, Whole
Health coaching, and Complementary Integrative Health (CIH) as well
as evidence-based strategies to effectively support patients in
health behaviors that prevent illness and promote
well-being.
- Develops training materials that addresses issues of social
determinants of health, cultural competence, diversity, and
inclusion in healthcare. Demonstrate knowledge and application to
the full scope of their license, education, and practice.
- Independently organize work, set priorities, and meet multiple
deadlines. The program coordinator oversees the daily operation of
the WH tele program, develops policies and procedures for program
operation, and prepares WH reports and statistics for facility,
VISN, and national use.
