Job Description:
PD Pharmacist I FRJOB SUMMARY
Participates in Medication Utilization Evaluations (MUE),
education, patient monitoring. Quality Improvement (QI) programs
and drug information activities of the pharmacy. Interprets
physician orders, evaluates patient profiles, makes appropriate
interventions as needed, enters medication orders into the
computer, compounds or prepares medication for patients as
prescribed by the physician, and practices and promotes the concept
of patient-focused care.
RESPONSIBILITIES
Essential Functions
* Demonstrates a commitment to the mission of Swedish Covenant
Hospital and demonstrates a service orientation and adheres to all
responsibilities and standards of the Hospital.
Participates in medication utilization evaluations (MUE),
education, patient monitoring. Quality improvement (QI) programs
and drug information activities of the pharmacy
Profiles medication orders via computer accurately and/or makes
appropriate changes or additions on the patient profiles, if
necessary.
Supervises and trains pharmacy technicians.
Ensures compliance with patient quality standards and maintains
quality assurance documentation including medication errors and
clinical interventions.
PATIENT CARE/AGE SPECIFIC RESPONSIBILITIES AND QUALIFICATIONS
Makes appropriate clinical interventions according to patient age,
height, weight, sex and condition for appropriate indications and
dosing.
Monitors drug interactions, drug allergies, Adverse Drug Reaction
(ADR) and contraindications.
ACCESS TO PROTECTED HEALTH INFORMATION:
Computer system access is assigned based on manager's access level
request and on the minimum necessary as described in this job
description. The system administrator or manager of the department
that has ownership of system/module gives approval for level of
access. See Computer system Access control policy.
QUALIFICATIONS/BASIC JOB REQUIREMENTS
Two years of experience in hospital pharmacy. Person should be
educated regarding disease states, pharmacology, and infection
control and pharmacy law.
Have excellent verbal and written communication skills, as well as
an ability to train others. Have Expertise in keeping good customer
service and follows hospital pharmacy policy and procedure.
Skills required: computer experience, ability to function with
minimal supervision and ability to utilize professional knowledge
to benefit patient care and to assist medical personnel.
Requires expertise in septic techniques and hospital pharmacy
policy and procedure.
Understands third party charge adjudication / reimbursement and
patient co-payment procedures.
LICENSE/CERTIFICATION
Graduate of an accredited college of Pharmacy - B.S. or PHARM.D
Currently licensed as registered pharmacist by the State of
Illinois Department of Professional Regulation
MANAGEMENT RESPONSIBILITY
Supervising the Pharmacy in the absence of the Pharmacy Supervisor
and training Pharmacy Technicians as needed.
INTERNAL AND EXTERNAL CONTACTS
Internal - Pharmacists, Technicians, Physicians, Nursing,
Transportation and Patients.
External - Pharmaceutical Sales Representatives and other
pharmacies.
ENVIRONMENT
Spends a majority of work time standing and walking in the pharmacy
(95%), with some sitting (5%).
Includes some moving or lifting of objects (up to 30 lbs.) on a
regular basis (0.5% of time). Includes about 80% of time spent
using a computer keyboard and communicating with patients, other
health care professionals regarding physician's orders and
prescriptions.
Includes about 65.7% of time using a computer keyboard and
communication with other health care professionals regarding
physician's orders and prescriptions.
Exposed to possible injury from chemicals, oncolytics, needles or
equipment (1-2%). Exposure to airborne germs when patient
counseling (less than 5%).
Other environmental conditions might include eyestrain and exposure
to other computer side effects, such as radiation and high-pitched
noise. Also, exposed to constant noise from printers and phones,
etc. about 50% of the time.
