Learn more at focalpointchicago.org. # Position Summary REPORTS TO: Supervisor # Manager of Med Surg, Chief Nursing Officer Position Status: Full-time Direct Reports: N/A Work Location: on-site Position Purpose: The Licensed Practical Nurse, provides quality patient care utilizing the nursing process under the direct supervision of a Registered Nurse and or Licensed Physician. Essential Duties#(list in order of importance including measures) Participates in assessment/reassessments of patient(s) by observing, collecting, recording and reporting objective and subjective data in an accurate and timely manner. Reports the condition and/ or change in condition of the patient(s) as well as signs and symptoms that deviate from normal health status.# 10% Assists in developing the nursing plan of care.# 10% Assists in implementing the nursing plan of care within the concepts of the Licensed Practical Nurse curriculum with consideration for safety in practice; according to established prioritization of needs; documenting and communicating nursing interventions and responses of patient(s) to care.# 60% Assists in evaluating patient(s) responses; document and communicate evaluation data to appropriate health care team members; contributes to the modification of the nursing plan of care on the based on ongoing evaluation of the patient(s).# 10% Collaborates with team members by sharing revelent information; demonstrating initiative, and supporting team decisions and the work life of the department.# 5% Performs other related duties as assigned within scope of practice after demonstrating required competencies or completing requisite training.## 5% # General Job Requirements (List any required knowledge, skills, experiences or training, including length of time necessary for proficiency, required to perform the job.) Position Summary

* REPORTS TO: Supervisor & Manager of Med Surg, Chief Nursing Officer
* Position Status: Full-time

* Work Location: on-site
