Job Description Registered Nurse (RN) – Home Health/Visiting Nurse - This is a direct hire, fulltime role - This is a salaried, 40 hour per week opening. Hours are 8a-5p, Monday – Friday. NO weekends, evenings, or holidays! - Nurse will perform home healthcare in the North Shore area in Chicago. North of Lincoln Square, south of Waukegan, and east of Palatine is the target region. Local candidates are required - Candidates must be an RN licensed in Chicago and have recent bedside care experience - Pay is $90,000-99,750K/year total compensation. As a Home Health/Visiting Nurse Registered Nurse, you will have the unique opportunity to provide individualized care, assess clinical and environmental needs, and support patients and their families through education and emotional support. - Serve as a vital member of our care team, delivering personalized care to patients in their homes. - Play a crucial role in assessing clinical and environmental needs, fostering a healing environment, and supporting patients and their families through education and emotional support. Primary Responsibilities - Provide high quality care management and acute care to patients enrolled in the Primarily Home program. - Conduct comprehensive in home assessments and develop individualized care plans tailored to each patient's needs. - Foster a healing environment by spending additional time with patients, assessing their clinical and environmental needs. - Act as a key educator, offering ongoing compassionate and empathetic support to patients and their families, ensuring they have the resources and understanding to manage their care at home. - Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes. Qualifications - Associate degree with 3 years of relevant experience or the knowledge, skill, or ability to succeed in the role. - Registered nurse in good standing in the state of Missouri and/or Illinois, with eligibility for licensure in other states. - Must have or obtain Basic Life Support (BLS) certification within 30 days of hire. - Valid driver’s license, automobile insurance, and access to reliable transportation for patient visits. Preferred Qualifications - Bachelor's degree from an accredited program with a major in nursing. - Experience in acute and home care is a plus. Working Conditions - The role may require driving a vehicle, transferring patients, lifting or carrying medical supplies, operating complex and delicate equipment with precision and accuracy, using a personal computer, and frequently interacting with care providers and patients. Additional Job Responsibilities - Conduct in home assessments of functional abilities and environmental needs. - Consult and coordinate with other team members. - Administer medications and treatments prescribed by physicians (e.g., blood draws, IV antibiotics, IV fluids, wound care). - Consult with members on their medications and durable medical equipment, review care plans, address home needs, and connect members to community resources. - Collaborate with primary care physicians and other providers to ensure continuity of care. - Work with members, providers, and caregivers to ensure positive outcomes during care transitions. PAY RANGE AND BENEFITS: Pay Range- $90,000-99,750K / year range.
