contributing positively to the community! Responsibilities About
the Role During visits that may last up to one hour, our providers
assess and observe a member’s current health status, medical
history, medication compliance, social circumstances, and other
potential risks. This approach provides invaluable insights into a
member’s overall health that are often missed during standard
office visits. The Matrix Comprehensive Health Assessment (CHA)
enhances care quality and allows us to address multiple care gaps
in a single appointment. Our providers are responsible for:
Conducting assessments for adults and geriatrics, which include
reviewing medical history, diagnoses, treatment plans, health
education, physician referrals, case management referrals,
follow-ups, and thorough documentation in accordance with our
guidelines. Administering and ordering necessary point-of-care and
lab-based tests. Collaborating with physicians, case managers,
social workers, family members, primary caregivers, and other
medical personnel as needed. Working alongside Primary Care
Physicians (PCPs) to provide patient education and necessary
follow-ups. Delivering services at the member’s residence, whether
in their homes or in Skilled Nursing Facilities. Benefits of
Working with Us Consistency – While assessments are conducted from
Sunday to Saturday between 8 am and 7 pm, our full-time providers
generally work Monday to Friday from 8:00 am to 5:30 pm.
Competitive Pay – Enjoy an annual base salary ranging from $105,500
to $168,400 , determined by market analysis and experience.

Additional Licensure – If business needs arise, we will assist you
in obtaining additional state licensure and credentialing in
neighboring states or others you may wish to visit while conducting
health assessments. Qualifications Skills & Experience That Shine A
Master’s Degree or equivalent experience along with successful
completion of NP or PA licensure. Current NP/PA licensure in the
state of practice, including prescription authority or the ability
to obtain it as required. Board certification from NCCPA, AANP,
AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult
Gerontology Acute Care, Family, Emergency, or Acute Care
specialties. Current BLS, ACLS, or CPR certification. A minimum of
one year of NP or PA experience. Willingness to travel up to 80-90%
of the time, including overnight trips and weekends across the
assigned state and possibly neighboring states. A valid state
driver’s license and proof of adequate automobile insurance for
your state of residence. Strong computer skills and familiarity
with health/medical record software. Excellent verbal and written
