Job Description:
Coverage Needed : Start Date: As soon as credentialed End Date:
6 months from start date Schedule: Inpatient & Outpatient; 1 day
clinic Call: 1:4 Clinical Details : Required Procedures: Standard
interventional cardiology core procedures EMR: EPIC (experience
preferred, but training available) Support Staff: Medical
Assistants and Registered Nurses Patient Volume: 10-25 in clinic;
10-50 inpatient Requirements : Active IL license IL DEA or one
easily transferrable Board Certified CV must be in Month / Year
format Must have license in hand at time of submission, or proof
that the license application has been submitted Any gaps greater
than 30 days will require an explanation on the CV DEA must remain
in state to maintain privileges. Provider must be willing transfer
and maintain DEA in state for the duration of privileges being
active, or obtain a new in state DEA to remain in state for the
duration of privileges being active
