Job Description:
Ascension is seeking a dedicated Registered Nurse (RN)
specializing in Case Management to join our compassionate,
faith-based team. At Ascension, we are committed to innovating

responsible case management to support and coordinate the
integrated health services ensuring that patients get timely,
efficient, and effective care. Assessing and analyzing patient
needs and healthcare outcomes. Developing, implementing, and
evaluating individualized patient care plans. Facilitating
collaboration between healthcare teams to ensure continuity of
care. Ensuring compliance with healthcare regulations and patient
rights. Providing patient and family advocacy and education.
Managing resources to ensure cost-effective care and minimize
waste. Advanced knowledge of case management principles and
healthcare delivery. Proficient in clinical assessments and
developing care plans. Strong communication and interpersonal
skills. Ability to manage multiple cases and effectively prioritize
tasks. Experience with Electronic Health Record (EHR) systems.
