Date: ASAP About the Position Primary Care Locum Opportunity -
Chicago, IL | MD or NP Welcome Outpatient primary care clinic
focused primarily on adult and senior patients (65) Collaborative
care environment with support from Nurse Practitioners, Registered
Nurses, and Medical Assistants Monday-Friday schedule, 8:00 AM -
5:00 PM No call requirements Fast credentialing - approximately 30
days Manage common chronic conditions including diabetes,
hypertension, hyperlipidemia, CHF, mild cognitive impairment, and
coronary artery disease history No procedures required Opportunity
to practice in a team-based outpatient setting in Chicago with a
steady patient population and strong clinical support
