Job Description:
Full-time Description Envision Unlimited is a full spectrum of
care organization serving people with disabilities from infants to

Innovative services offered across Chicagoland and Central Illinois
include community living options, novel day programs, behavioral
health services, employment programs, respite for families, and
foster care for children with disabilities. Purpose: Evaluate and
provide training to residents in daily living and social skills in
order to assist them in achieving and maintaining their maximum
level of functioning and independent community living. Essential
Responsibilities , include but are not limited to: Evaluate and
provide training to residents in daily living skills including, but
not limited to: personal hygiene and grooming (which may involve
physically assisting individuals to complete the tasks),
housekeeping, laundry and clothing care, meal planning and
preparation, money management and budgeting, socialization, and
recreation leisure activities. Implement group and individual
programs which promote growth and development toward independent
living skills for residents. Assist residents in preparation of
daily schedules to include performance of household chores, social
activities, and other meetings and appointments. Complete and
ensure the timelines of all assigned documents and forms to include
daily progress reports, CILA shift reports, van log sheets,
incident reports, MAR sheets, etc. Monitor maintenance of living
facility to assure safe, clean and healthy environment for
individuals. Perform cleaning and housekeeping duties such as, but
not limited to: vacuuming, dusting, sweeping, and mopping, monitor
and assist residents with daily bedroom maintenance. Monitor
residents to ensure wellness and safety. ( deleted enforcing
curfew) Supervise resident’s schedules and activities to ensure
timely arrival to day program or other appointments. (deleted job
site) Supervise and assist residents in taking their medication in
accordance with program guidelines. Attend and participate in any
staffing's as requested . Assist in planning and supervising
recreational and social program activities. Plan menus and assist
with meal preparation as required. Monitor /support and observe
residents with special dietary needs. Attend and participate in all
staff meetings. Respond to medical and behavioral emergencies
following appropriate Health and Safety , Safety Care, First Aid,
CPR, and other procedures as necessary. Monitor and assist
residents with personal grooming and appearance as necessary.
Immediately report any medical/behavioral issues or emergencies or
other unusual incidents to House Manager, Administrator, QIDP, and
Registered Nurse Trainer. Complete training on the Skills System
and ability to implement with all members. Must be able to pass
Safety Care training, demonstrating physical ability to perform ALL
stability holds and leg wraps. Other Responsibilities: · Transport
and accompany residents to appointments and other services,
including work centers, as needed. · Check all rooms upon arrival
to ensure that all residents are home and regularly monitored to
ensure wellness and safety. · Check and secure all doors and
windows to ensure building safety ( delete and activate alarm
system nightly) · Perform other related duties as assigned. OTHER

functions. EDUCATION, SKILLS, and/or EXPERIENCE: Two years of
college or one year of college plus two years of full-time related
work experience or high school diploma or equivalent plus five
years full-time related work experience required. Completion of
40-hour training program provided by BCBA staff based on the RBT
List and ability to pass the RBT Competency Assessment per BCBA
guidelines within 90 days of completing the RBT training. Must have
proof of Direct Service Professional (DSP) training and pass TABE
test prior to hire . Must pass a fitness for duty exam. Must
complete Safety Care training. Must have a valid driver’s license
and ability to drive agency vehicles. PHYSICAL DEMANDS While
performing the duties of this job, physical requirements may
include standing, walking, and sitting; seeing; hearing and
listening; clear speech; dexterity and use of hands and fingers.
Must be able to push, pull, and lift 50lbs. Candidate should be
able to comfortably bend, sit and get up from seated position.
Candidate will also need to be able to perform physical
intervention techniques (to include but not limited to 1- or
2-person transfer, kneeling, blocking, moving away from flying
objects, etc.). Candidate should not have any physical limitations
that will prevent them from performing ALL essential and physical
duties of this job. The physical demands described here are
