A national position reporting to the CMO, this individual will be responsible for ownership of organizational performance against key clinical and operational metrics related to the acute care needs of engaged patients. This Chief of Acute Care (CAC) will be responsible for the performance and continued growth of the Landmark First (LM1). This includes the LM1 telephony program and the LM1 facilitated telemedicine program. Working with the Landmark First Medical Director, National VP of Patient Access and other leaders, this leader will ensure efficient call routing and clinical and non-clinical staffing needs. This leader will also focus on continued collaboration and seamless transitions of care between Landmark First and the market teams. This leader will also be charged with ensuring the reliability, quality and impact of all acute response clinical models, some of which operate in the markets. The leader will balance the needs of our providers against the need to be available to patients and families at unscheduled moments. This leader will ensure that acute episodes of care are performed in a manner that does not disrupt or distract from longitudinal care team relationships and efforts.

Primary Responsibilities:

  • In partnership with other operational and clinical leaders, formulate a comprehensive plan to meet the acute unscheduled needs of engaged patients
  • Ensure the quality of the care delivered and the reliability the information flows in the Landmark First clinical call and treatment center
  • Ensure the quality of the care delivered and the reliability of the information exchanges between providers for all field-based acute visits
  • Development, initiation and operations for all acute clinical care needs that requires ongoing care and monitoring in the home
  • Create a set of field-based urgent visit solutions that balance provider workload with the need to be responsively in patient's homes in
  • Build a quality oversight and review program for all facets of the acute visit function with regular data dashboards available to front-line staff and leaders
  • Build and oversee a vendor management and review function for all external partnerships in the acute care domain
  • Working with the training and market leadership teams, develop a robust orientation and ongoing education plan for all acute care roles
  • Oversees and participates in the development, documentation, implementation, and interpretation of necessary acute care medical policies, procedures, and guidelines
  • Working with legal and compliance to understand and function within state regulations germane to acute care services
  • Recruit, hire, manage and retain a high performing acute care provider team
  • Interact with market leadership regularly to mitigate any acute clinical quality or process issues and collaborate on service enhancements and best practices
  • Appropriately escalate clinical and management concerns to the Chief Medical Offer
  • Appropriately address complaints with the Quality committee and the markets affected

    Required Qualifications:
    • Degree from an accredited medical school with currently active Board Certification
    • Experience in Emergency, Internal, Family, or Geriatric Medicine preferred with clinical care practice background including experience caring for highly complex patient populations
    • Experience developing and managing clinical programs, focused on the chronically ill and managing medical expense
    • Experience in quality assurance, and program development coupled with solid implementation skills
    • Leadership experience in either a fully delegated medical model or as a leader and clinician in a complex care delivery model
    • Deep experience in informatics and analytics
    • Possess in depth knowledge and experience on acute care models for complex patients in the community and home settings
    • Expertise in geographically distributed clinical teams with a solid focus on managing performance though metrics and quality indicators

            Colorado, Connecticut, Nevada, or New York City Residents Only: The salary range for Colorado/Connecticut/Nevada/New York City residents is $319,351 to $376,000 .
