Licensed Therapist ACT (Assertive Community Treatment)POSITION
IS ELIGIBLE FOR HIRING BONUSDue to eligible grant funds, this
position offers a $2000 hiring bonus. The bonus will be paid to the
hired candidate half after 30 days ($1,000) and half upon the
successful completion of 6 months of work ($1,000). Employees must
be in good standing to be eligible for hiring bonuses.Starting
salary: $60,000 plus increases for license and experience ($65,000

location: Elk Grove Village, ILCommunities served: Mainly Northwest
suburbs, approx. 15-mile radius from Elk Grove Village.Job Scope:
This position acts as the licensed clinician working on an ACT
(Assertive Community Treatment) team in the Northwest Suburbs.
Provide assessment and treatment planning utilizing IM+CANS tool.
Support ACT team members to provide a range of individualized
treatment services to individuals with serious mental illness.Proof
of COVID-19 vaccination required. Accommodation for medical and
religious exemption will be considered. Weekly testing required for
approved vaccination exemptions.Responsibilities include:Complete
semi-annual IM+CANS assessments and treatment plansProvide
individualized treatment services including therapy, skill
building, and crisis management and stabilizationAct as lead
clinical staff alongside ACT supervisor to provide clinical
treatment to ACT clients per Rule 140 modelSupport adults with
mental illness to identify and develop coping skills and create
crisis mitigation plansProvide services to assist client in
improving their ADLs and independent living skillsCoordinate with
ACT nurse, ACT psychiatric provider, and KYC's Front Door Diversion
and Colbert & Williams transition teamsMeet productivity target
estimated at 45% billable serviceEducation, Experience, and Other
RequirementsRequired Masters Degree from an accredited college or
university in social work, psychology or a related
disciplineRequired LCPC/LCSW or equivalent that meets IL Rule 140
LPHA designation, or ability to obtain within 6 monthsPreferred 2
years of experience working with individuals with serious mental
illnessPreferred 2 years working in non-traditional treatment
environments i.E.In-home, residential, hospital, nursing facility,
or community settingsBasic Technology and computer proficiency:
knowledge of Word, Electronic Medical Records systems, smartphone
messaging and applications, and team collaboration toolsValid IL
Driver's License and daily access to a maintained/insured
