- - - - - - - - - Bachelor's degree required, Master's degree preferred in Human Services Field (Social Work, Psychology, Counseling or other related field).

Job Summary
- - - - - - - -The Social Service Director Assistant will be responsible for performing assigned social work duties and responsibilities within the facility in accordance with Federal, State and local guidelines and regulations.
- - - - - - - - - Must be able to assess and document residents' needs and formulate treatment plans
- - - - - - - - - Conduct/oversee and complete assigned initial and all ongoing assessments and MDS, CAAs
- - - - - - - - - Conduct biopsycho-social assessments of newly admitted residents/initiate a therapeutic relationship
- - - - - - - - - Facilitate/participate in discharge planning
- - - - - - - - - Make supportive visits to residents and provide appropriate counseling, case management and crisis intervention services
- - - - - - - - - Must act as resident advocate/a liaison between the resident and his/her family, the facility and community agencies
- - - - - - - - - Participate in the IDT members meetings as assigned
- - - - - - - - - Resolve resident concerns and facilitate resident, family, and staff education.
- - - - - - - - - Perform other related duties as assigned
