PACE is a collaborative, interdisciplinary approach which coordinates all levels of medical and social needs, including prevention, wellness, socialization, primary care, and access to specialists and other services.The Activities Coordinator plans, develops, implements, and oversees recreational therapy services for participants in PACE (Programs of All-Inclusive Care for the Elderly). Responsibilities include coordinating, supervising, and implementing all PACE center activities and social programs for frail elderly and functionally impaired older adults at the PACE Center in an integrated program setting. The Activities Coordinator will be responsible for the development and implementation of daily, monthly, and special events, outings, birthday celebrations, etc. The Activity Coordinator reports to the Site Director. Primary Duties and Responsibilities:

  • Conducts an initial assessment and scheduled and as-needed reassessment for all PACE participants.
  • Performs all functions and duties as required as a member of the PACE Interdisciplinary Team (IDT), including, but not limited to, participation in IDT meetings, family conferences, grievances, service determination requests, CMS audits.
  • Coordinates with IDT to assist in developing individualized plans of care for each PACE participant.
  • Plans and implements creative, therapeutic activities for participants' care plans, including posting of weekly activity calendar.
  • Offers therapeutic activities daily which are appropriate for PACE participants. Includes options to accommodate activities for individuals with special needs. Uses group and individual-oriented interventions to maximize participants' cognitive and functional abilities.
  • Organizes PACE Participant Advisory Council to elicit participant feedback as well as gather suggested programming and activities for the PACE Center.
  • Participates in quality improvement activities of the PACE center and other designated projects.
  • Trains staff on new recreational therapy activities and programs.
  • Documents participant involvement in activities and revises care plans as needed.
  • Actively participates in team discussions offering feedback, ideas, and suggestions both on a day-to-day basis and in staff meetings.
  • May plan special events (e.g. holidays, celebrations, special programs) periodically.
  • Engages with community centers, fitness programs, schools, etc to coordinate activities at the PACE Center.
  • Coordinates volunteer activities at the center. Requirements:
    • Bilingual (preferred).
    • Reliable transportation.
    • Flexible schedule to meet the needs of participants.
    • Ability to physically access any home (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning).
    • Bachelor's degree in recreational therapy or related field.
    • CTRS (Certified Therapeutic Recreation Specialist) required.
    • Minimum 2 years of professional therapeutic recreation work experience preferred.
    • Minimum of one year experience working with the elderly population preferred.
    • Experience with supervision of staff preferred.
    • Must be certified in CPR/BLS (Basic Life Support) by the American Heart Association.
