Therapist Program: Adult Outpatient Broadway (AOB) Reports to:
Supervisor of Art and Group Therapy Services Who we are: C4 was

recovering from mental illness and emotional trauma are able to
live, work and thrive in the community. Purpose: Adult Individual
Therapists provide Trauma Informed individual therapy as well as
coordinate and provide rehabilitative services designed to increase
clients’ abilities and resources necessary for community living,
socialization, work and recovery. Core activities include cognitive
behavioral interventions, problem-solving, programming to reduce or
ameliorate symptoms of co-occurring disorders and other
rehabilitative services and interventions. Minimum Qualifications:
MA/MS in Social Work, Psychology, or a related field is required
Clinical experience working with individuals living with chronic
mental illness and co-occurring disorders Excellent interpersonal,
verbal and written communication skills Competencies (Minimum
Skills, Knowledge and Experience): Ability to be respectful of the
diverse cultures of the people served and to provide culturally
appropriate, competent and individualized treatment in accordance
with each client’s age, gender and gender identity, sexual
orientation, race, ethnicity, religious beliefs, and culture
Clinical competencies, including Motivational Interviewing
techniques, CBT and other clinical interventions proven successful
in the treatment of trauma, acute and persistent mental health
challenges aligned with training mandated by C4 Understanding of
personal and professional boundaries Ability to closely collaborate
with other members of a multidisciplinary treatment team Ability to
plan, develop and manage multiple projects Solid organizational
skills Time and stress management skills Communication skills
including interpersonal, teamwork, and collaboration skills Sound
clinical judgement Conflict resolution and diplomacy skills
Computer literacy Excellent problem-solving skills Excellent
interpersonal, verbal and written communication skills Ability to
plan, develop and manage multiple projects and case loads
Proficiency in Windows environment Duties and Responsibilities:
Provide individual therapy to adult clients Co-facilitate and/or
lead skill-building and psychoeducational groups in support of the
program’s overall purpose at times when no other Group Therapist is
able, in order to ensure continuity of services. Maintain
compliance at all times with clinical documentation, timelines and
agency policies and procedures including meeting required
timeframes for documentation and service notes Complete an IMCANS
assessment resulting in the identification of the individual's
mental health service needs and recommendations for service
delivery Update the IMCANS every 6 months ensuring the Treatment
Plan continues to support continued Individual and/or Group
Services Regularly assess clients’ functioning, strengths, and
needs, as well as indicators of substance abuse Apply appropriate
behavior and emotional management techniques Provide culturally
appropriate, competent and individualized care in accordance with
each client’s age, gender and gender identity, sexual orientation,
race, ethnicity, religious beliefs and culture Facilitate
psychiatric hospitalization when indicated Work collaboratively and
cooperatively with other members of the treatment team Maintain
positive and mutually respectful relationships with other human
service providers, entitlement agencies, health care vendors, and
the community Report to supervisor and actively engage in weekly
supervision and/or weekly group supervision Seek consultation with
various C4 treatment teams and/or clinicians whenever necessary or
requested by supervisor Maintain compliance with confidentiality
policies and procedures at all times Other duties as assigned by
supervisor
